The original issue regarding guest not asking for email address is resolved. I'm running the latest 4.0.5. Regarding all of the dialogs in 'Orders Statuses' however, they still do not show the 'Email (Others)' field - only an 'Email (Customer)' field contrary to what you indicated in your post above. Are we looking at the same screen? This screenshot is still applicable: https://talos4.net/images/OrderStatusPending.png as what my system currently reflects.

Hi, try to enable standard editor and take the screenshot of whole page - to the footer of the page.
Enable php error reporting and debug mode to see if the page script is not cancelled by some php error.
