Menu - List of Files in Category view 404 Error

I have menu items using the List of Files in Category view menu type that work fine. Others i receive a 404 error when the pages load and it adds "/Categories" either at the end of the url or in the middle. Some of those menu items worked fine a few days ago but don't understand why its adding that extra bit to the url.

Hi, which component and which version you are using? Can I see the site where this occurs?

I am using the phoca download component. I actually figured it out. When we would make a change to a category, or chose a new category it would add that extra bit to the url. I went back into the menu item, clicked the Rebuild option and it removed the"/Categories". Everything seems to be working fine, but it was only the phoca download menu items that were having the problem.
