An Error Captha on Check Out page

Hi, does anyone know how to fix a 'Captcha Error" message? It kept showing on check out page after guest/user filled-up the forms after checking-out, even though we could not see the box nor random object for inputting the captcha. Thanks in advance!

Hi, did you set the keys for the Captcha in Options?
