Display Specific Layout = No
Display Bootstrap 3 Layout = No

Hmmm, really hard to say what can be wrong there. Are you able to install Phoca Download somewhere on test server. Because with default settings the image is normally displayed there

Ok, i will try to install a default joomla site and install only phoca download to try.
