Problem with Image into pdf export

I have installed Phoca Pdf for formatting my output when download website page in pdf format.
Into my page I have some images, and where these aren't, the Phoca Pdf goes.
But if there are the images, return this error :

TCPDF ERROR: [Image] Unable to get image: /web/htdocs/......

Why?

Hi, seems like image which is pasted into the article cannot be read by TCPDF - try to check all the permissons of files and folders of your server, try to check e.g. if there is no server protection to access images.

I have another two problem.

There isn't a Title of My WebPage in pdf body but only like title of pdf file.
And with Microsoft Exlorer Phoca Pdf doesn't work!!!
Why?

I have same problem with the plugin in joomla 1.6, and I changed the permissions to 777 (read , write , execute for everyone) but problem remains and the same error appears so i think it's not permission error, what else should I do?
