Same problem here.
Phoca Maps 2.0.6
Phoca Maps plugin 2.0.5
Joomla 2.5.14
Example: demo.libelnet.eu/_libelnet-basisinstallatie/contact
What you see at this page is an ugly templatem, but it's only for testing. First you see an article (Contact) and in that article I call 2 plugins (Phoca Maps and RSForms). After that I have 2 modules (Demo Maps and Demo RSForm). Both Custom HTML and both call the plugin wit {code}.
Some conclusions:
Phoca Maps plugin works in article
Phoca Maps plugin doesn't work in Custom HTML module
Looks like the problem is not in Joomla, because the other plugin works both in article and module.
Could you help me finding the problem?

Hi, Phoca Maps plugin is standard Joomla! plugin, it is a content plugin, which means, it is displayed in content. If you enable "prepare content" in module - Custom HTML module, then it as standard plugin is rendered there.
Testing now, see no problems, see the screenshot.

I've never known that it's necessary that "prepare content" is enabled.
