Add New Category Problem

I am using Phocadownload version 2.1.6 with Joomla 2.5. Recently I have shifted my site to another server and suddenly this category page display issue is coming up. Whenever i try to add new or edit any existing category the page could not display complete fields. The same problem is coming up with Phoca Gallery component as well. I have tried everything to solve this issue but no solution. I am also attaching a screenshot for better understanding.

Hi, mostly this is caused by PHP error, enable php error reporting on your server to see what is wrong there: https://www.phoca.cz/documents/16-joomla ... rs-on-site
