If you add the new picture but with the same name, you need to remove the thumbnails. You can do it e.g. via media manager or FTP, just remove the thumbs in thumbs folder (for the specifed category) or you can use the recreate function, see image (Admin - Phoca Gallery - Images list).
This function will delete the image thumbnails and create new (and if new original image is uploaded, the new thumbnail will be created from new image.

On a similar note - what if you go in and change the size of the small images (say from 50px to 100px) and then want to recreate all thumbnails - is there a way to do this for all images?
It can be a big task if you have to click to delete and recreate thumbs for each image - for the moment I'm deleting them through FTP.

Yes the method to delete all thumbnails via FTP or some manager like media manager is the best solution... no other method for multiple deleting exists in Phoca Gallery ...
