Hi, while using Phoca PDF you need not to add plugin code to article, the pdf icon is rendered automatically, see:

Without any knowledge about the use of PhocaPDF but maybe there are template overrides for the article rendering. So please check if there are folders similar to this:
/JOOMLAROOT/templates/YOURTEMPLATE/html/com_content/article/default.php
/JOOMLAROOT/templates/YOURTEMPLATE/html/com_content/category/blog_item.php
/JOOMLAROOT/templates/YOURTEMPLATE/html/com_content/featured/default_item.php
If one of these exists, you have to edit them as described in the "custom install" part of the documentation. But please keep in mind that the code of the overrides could be different so maybe you have to modify it different.
