Markup lost when adding new file

Hello & thanks for an excellent component.

Just recently - when I go to add a new file the markup is list and I am left with ;
New File
Title
Alias
Category *
OrderingNew items default to the last position. The ordering can be changed after this item is saved.
Filename *
Select Filename
File - Play
Select Filename
File - Preview
Select Filename
Icon

etc, etc,
Any suggestions on how to fix this?

Hi, sorry, I don't understand what the problem is? Which markup you mean?
