On the admin site, when pressing the New button in Categories or Images, I get a simple html form without any formatting and with no controls. The standard Joomla interface with buttons, menu, etc.is missing.

So, no way to create anything.

I'm using Joomla 2.5.8 and Phoca Download 2.1.8. Looks like a .css/.php is not loading or missing. Any assistance would be helpful.

Hi, this seems like the loading of html is broken by some php error which is not displayed, try to enable php error reporting to see the problem:
