File selection quits working in new file download setup

After installation Phoca Download file selection page works (insert button) correctly for a varied period of time. (month, 6 months, etc) Then for no reason the file selection page (insert button) quits working after browsing, uploading and seeing the file I cannot select it. I must then reinstall phoca download module. What am I not doing correctly?? The file does appear in the public html/phocadownload directory and the www/phocadownload directory but still nothing happens when I try to select it on the file page. Now, after reinstalling and even updating the file insert button does not work at all. If I manually type the name of the file for downloading and preview in the appropriate box everything else works normally. Any help would be appreciated. Joomla 1.5.21 , Phoca 1.3.9.

Hi, not sure what error this can be - try to discover what was the changes after them it has stopped to working
Try to enable debug mode and php error reporting, maybe you will get some more info, maybe some error which prevents from running it Ok

After manually entering about 20 download files the file selection feature started working again. I do not know why but will try to look at php error reporting the next time it stops working. Thank you for your reply.
