404 Component Not Found - Cannot Install Phoca Maps

Hello!
I am trying to install com_phocamaps_v3.0.1.zip into my Joomla 3.3.6 installation. This Joomla installation was a fresh build, not a migration.
I am getting this error () only with Phoca Maps. I have successfully installed Phoca Downloads, and many other extensions, so this leaves me with the idea that it is related to Phoca Maps specifically.
The Phoca Maps component folders were added, but no tables. I added the tables manually as instructed on your website, matching the table prefix that I already had on my site. That went fine. BUT, Phoca Maps does not show up in my Components list, nor does it show up in the Extension Manager.
Do you have any recommendations? I really want to use this extension, but for the life of me, I can't get it installed. I have followed the instructions on your site and it will not show up. My next option is to try to manually uninstall it.
I also called my host to see if it was a file/folder permissions issue. There were no issues there and after resetting the file/folder permissions, the Phoca Maps issue did not resolve itself.
Any suggestions? Please? I need to get this site done ASAP and this is one of the last things holding me up.
