Did you add the code into the php code (so the icons will be displayed) try to check all steps you have done:

i added the code in the php-code. In your description you wrote:

„Paste the following code (without tags) after row 148 (after $tab->endTab(); and before $tab->endPane();)“.

In my PHP-Code is the „(after $tab->endTab();“ in row 170, not after 148.

Please look at my screenshot.

The problem is I don't know how to advice, I have tried it on my localhost and there were no changes so the code should still work like it worked. No idea where can be the problem on your site

It should be paste after row 170, try to check the steps again (I have fixed the row in the guide):
