What is the URL (with or without the domain name) of the page this error occurs?

I have the same problem at the url:
/administrator/index.php?option=com_phocamenu&controller=phocamenuconfig&task=edit&type=1&typeback=group
when i Try to insert a new header and a new footer in setting of daily menu.

I use phoca reastaurant 1.1.1 lite

I have used pr lite 1.1.0 and it's ok.
I have unistalled 1.1.0 and installed 1.1.1 for compatibility with phoca_gallery 2.6.2 and i have this problem.

I have find an error.

During installation, the component do not create the table of mysql database.

I tried many installation and now it's ok.
