PhocaDownload Preview button not working after upgrade to Joomla 3.6.2

I have just migrated from J2.5 to 3.6.2, following the Phoca guide. Everything seems to be working except for Preview.

The Preview buttons used to be red but now they are blue. When a Preview button is clicked, I receive an error message saying that' the requested URL was not found on this server' and 'Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocuemnt to handle the request'.

Any ideas as to what the problem could be?
