Just installed the plugin to try out, 2.5.9. No matter what I enter for imagecategoriessize it always displays the small thumbnail.

Figured it out. You have to use a number, not the name as stated in the documentation.
{phocagallery view=categories|imagecategoriessize=1}
not
{phocagallery view=categories|imagecategoriessize=medium}
{phocagallery view=categories|imagecategoriessize=1}
not
{phocagallery view=categories|imagecategoriessize=medium}
